Private Mentoring Journey with Abelina Application Form

I hereby acknowledge that I understand the above & give my consent for Abelina to contact me via email and phone*

PLEASE NOTE: If you choose a monthly or weekly payment plan option, a payment deposit of $100.00 AUD is required at least 48-hours before your initial session with Abelina to secure your journey. This amount will then be deducted from the final payment of your plan. Please confirm you understand this.*

I understand that this Private Mentoring with Abelina is designed as a transformative self-development journey and that it is NOT intended as a replacement for professional therapy.*

I understand that Abelina is not acting in her capacity as a registered psychologist, that the Private Mentoring Journey does not constitute psychological treatment, mental health intervention, diagnosis or clinical advice, and does not establish or continue a psychologist-client therapeutic relationship.

I understand that if I have any physical, mental or emotional concerns that may impact my engagement in this journey that I inform Abelina of this who will re-direct me to appropriate support (e.g. GP, psychologist, psychiatrist or other health professional).*

I understand that this Private Mentoring Journey includes physical activity such as yoga, somatic movement or dance, which has certain hazards and risks due to physical exertion. If I have any current concerns around my physical health or a history of physical health concerns (e.g. injuries or pain) that may increase further risk or my ability to participate in these practices, I will consult with a doctor or other appropriate health profressional first for clearance.*

I understand that as part of Abelina's qualitifaction as a Certified Yoga Teacher, physical touch may be offered and used for any physical adjustments during a yoga practice or for therapeutic purposes, and that I have every right to decline this if I do not feel comfortable. I understand that Abelina will always check for my verbal consent in sessions prior to using physical touch.*

I understand that support is available to me from Abelina via email (Mondays to Fridays 10am to 5pm AEST) throughout the entire agreed duration of the Private Mentoring Journey. If there are any immediate concerns around my health or if there is a risk of harm to myself or others, I understand that Abelina will re-direct me to appropriate health professionals or 000 in cases of emergency. *

I understand that by participating in this Private Mentoring Journey with Abelina that I am doing so at my own risk, and that I hereby agree that I do not hold Abelina Wolf as the mentor and business owner liable for any incidences that may cause injury (physical, emotional or mental), loss, damage or death. *

I understand that the initial payment for this Private Mentoring Journey (whether it is payment in full or the $100.00 AUD deposit) is non-refundable and that by completing this application form I am making a commitment to attend the Private Mentoring sessions. *

I understand that payments (dependant on which payment plan I have chosen) are to be completed at least 48-hours prior to the next booked session with Abelina. I understand that payments will be completed via bank transfer through invoices provided via email.*

I understand that these Private Mentoring Sessions are not covered by Medicare in Australia and that a rebate would not be available. *

I understand that if I need to reschedule a session, that I provide Abelina with as much notice as possible. I understand that if I cancel or reschedule a booked session less than 24 business hours prior to the session that a cancellation fee of $100.00 AUD applies.*

Please note that in-person Private Mentoring Sessions will be held at Abelina's home address. I understand that throughout my Private Mentoring Sessions, certain boundaries will need to be respected and that I can only attend the property for scheduled appointments agreed with Abelina, and that if I was to present needing support outside of our agreed scheduled appointment times, Abelina will redirect me to appropriate supports such as emergency services.*

In the event that any dispute arises out of this agreement, I agree that the dispute shall be resolved by mediation before a mutually agreed and selected mediator by both parties. I agree that in the event that a mediation fails to resolve the dispute, the matter shall be resolved with an arbitrator. *
